What the lawsuit really means, how rare reactions occur and whether other fillers such as Hyalmass are similar

Social media loves two things: a dramatic headline and a frightening statistic.
A recent Instagram post claimed that AbbVie had been hit with the “largest class action in the history of injectables” and warned that Juvéderm fillers could cause serious granulomas and disfiguring facial nodules.
There is a genuine lawsuit behind the story.
There is also genuine science showing that delayed inflammatory reactions can occur after hyaluronic acid filler.
However, the social media version moves rather quickly from “a lawsuit has been filed” to “millions of people may be disfigured”.
Those are not the same thing.
What has actually happened?
In June 2026, a proposed class action called Garcia v. AbbVie Inc. was filed in the United States.
The claimant alleges that AbbVie did not adequately warn patients about the possibility of delayed granulomas following treatment with certain Juvéderm fillers.
At the time of writing:
- the allegations have not been proven in court;
- AbbVie has not been found liable;
- the proposed class has not been certified;
- and there is no evidence that millions of patients have joined the case.
A lawsuit is a legal allegation, not the result of a clinical trial or a court judgment (Garcia v. AbbVie Inc., 2026).
There is another important detail.
The Instagram caption singled out Vycross fillers such as Volbella, Vollure and Voluma. However, the filed complaint concerns Juvéderm Ultra, Juvéderm Ultra Plus and their lidocaine-containing versions.
The social media post therefore appears to have broadened the story beyond the specific products named in the case.
What is a filler granuloma?
A granuloma is an organised immune response that develops when the body attempts to isolate something it continues to recognise as foreign.
With dermal filler, it may appear as a firm or swollen area weeks, months or occasionally years after treatment.
But an important distinction is often lost online:
Not every lump is a granuloma.
Temporary swelling, product that can be felt beneath the skin, uneven placement, infection, inflammatory nodules and true granulomas are different things.
A true foreign-body granuloma is usually confirmed through a biopsy and microscopic examination. Simply feeling a lump does not establish that one is present.
How common are delayed reactions?
Delayed inflammatory reactions are recognised complications of hyaluronic acid filler, but published research generally describes them as uncommon.
A systematic review found that most retrospective studies reported delayed inflammatory reactions in fewer than 1% of patients. Genuine delayed hypersensitivity reactions appeared to be considerably less common (Turkmani et al., 2019).
One retrospective study followed 2,139 patients treated with Vycross fillers. Seven developed delayed-onset nodules, giving an overall incidence of 0.33%.
Only one of those nodules was biopsied and confirmed as a foreign-body granuloma. All seven patients’ nodules eventually resolved, either spontaneously or following treatment (Rivers, 2022).
That does not mean delayed reactions are trivial for the person experiencing one.
It means that describing them as something likely to affect enormous numbers of filler patients is not supported by the available evidence.
Is BDDE responsible?
Most long-lasting hyaluronic acid fillers are chemically cross-linked. This makes the gel more resistant to the body’s natural breakdown processes.
One commonly used cross-linking agent is 1,4-butanediol diglycidyl ether, usually shortened to BDDE.
The lawsuit presents BDDE as a possible contributor to granuloma formation. However, the scientific position is not that simple.
Current reviews suggest delayed reactions may involve several interacting factors, including:
- the physical and chemical properties of the filler;
- filler degradation products;
- infection or bacterial biofilm;
- injection technique and product volume;
- dental treatment or illness;
- immune activation;
- and the individual patient’s susceptibility.
Researchers have not established that residual BDDE alone causes filler granulomas (Lee et al., 2024; Baranska-Rybak et al., 2024).
What about Hyalmass?
At BONAVADA Aesthetics in Worthing (formally Mr Beautox), I use Hyalmass rather than Juvéderm, so this was naturally one of the first questions I asked.
Hyalmass and Juvéderm are not identical products. They use different manufacturing and cross-linking technologies.
However, they do share some broad characteristics:
- both are hyaluronic acid fillers;
- both are cross-linked to increase their persistence;
- and BDDE is used during their manufacture.
Manufacturer-supplied information describes Hyalmass as a cross-linked, biphasic HA filler and states that residual BDDE and endotoxin are reduced below its detection limits (MAYPHARM, n.d.).
That is reassuring from a manufacturing-quality perspective, but it does not make granuloma formation impossible.
Delayed inflammatory reactions are not necessarily caused by unreacted BDDE. Any persistent injected material can potentially produce a foreign-body response in a susceptible patient.
As of July 2026, I could not identify robust, independent, long-term research giving a reliable Hyalmass-specific rate for delayed nodules or confirmed granulomas.
That means we cannot honestly claim that Hyalmass has the same risk as Juvéderm.
We also cannot claim that its risk is definitely lower.
Should people with existing filler panic?
No.
The existence of this lawsuit does not mean that everybody with Juvéderm, Hyalmass or another HA filler should have it dissolved.
Most people who receive hyaluronic acid filler do not develop delayed inflammatory nodules or granulomas.
However, patients should know that delayed reactions are possible and should contact their practitioner if they develop:
- new or persistent firm lumps after the original swelling has resolved;
- unexplained swelling in a previously treated area;
- redness, heat or increasing tenderness;
- repeated swelling following illness or dental treatment;
- or symptoms that continue to worsen rather than settle.
Assessment is important because not every delayed lump has the same cause or requires the same treatment.
My evidence-based conclusion
The lawsuit is real.
So is the possibility of delayed inflammatory nodules and foreign-body granulomas after hyaluronic acid filler.
What has not been established is that Juvéderm will cause widespread disfigurement, that BDDE is solely responsible, or that millions of patients will develop serious complications.
The most accurate conclusion is:
Delayed inflammatory nodules and granulomas are recognised but uncommon risks of cross-linked hyaluronic acid fillers. The new lawsuit raises questions about how those risks were communicated, but it has not proved that Juvéderm is unsafe or that the problem is unique to one brand.
Different fillers use different technologies and should not automatically be treated as identical.
Equally, no cross-linked HA filler should be marketed as completely incapable of causing a delayed reaction.
That may not make the most dramatic Instagram headline, but it is what the evidence currently supports.
Frequently asked questions
Is every lump after filler a granuloma?
No. Early swelling, bruising, palpable filler and unevenness are not automatically granulomas. A true granuloma is a specific inflammatory diagnosis, often requiring biopsy confirmation.
Can a granuloma appear years after filler?
Delayed reactions can appear months or occasionally years after treatment, although they are uncommon.
Does BDDE make dermal filler dangerous?
BDDE is widely used to cross-link HA fillers. Current evidence does not establish that BDDE alone causes granulomas. Delayed reactions appear to be multifactorial.
Is Hyalmass safer than Juvéderm?
There is not enough independent comparative research to make that claim. They are different products, but both are persistent, cross-linked HA fillers and delayed reactions are theoretically possible with either.
